Housing and Outreach Services Manager

The Eau Claire County's Outreach Manager provides leadership, supervision, and strategic direction for the Community Outreach Team serving individuals and families experiencing homelessness, substance abuse, those experiencing symptoms related to mental health, and those dealing with other traumatic events including incarceration.

This role ensures trauma-informed, person-centered, and housing-focused services that promote stability, safety, and long-term housing outcomes through a collaborative community approach.Program Leadership and Clinical Oversight
  • Interprets and administers federal, state, and county laws, rules, regulations, and standards governing the CCS Program; ensures compliance with Wisconsin DHS 36 and other applicable regulations.
  • Provides motivational and trauma-informed leadership to supervise, coach, direct, and evaluate CCS staff.
  • Provides orientation, training, clinical consultation, and supervision to staff regarding client assessment, recovery planning, and service implementation.
  • Conducts and/or reviews brief, focused assessments to support rapid and appropriate service engagement.
  • Reviews and approves assessments, recovery plans, services, progress documentation, and discharge summaries.
  • Promotes evidence-based and recovery-oriented practices, including motivational interviewing, strength-based case management, and person-centered planning.
  • Supports staff in engaging individuals with complex and co-occurring mental health and substance use conditions.
Collaborative Community Engagement
  • Develops and maintains partnerships with behavioral health providers, primary care providers, housing agencies, reentry programs, correctional facilities, hospitals, law enforcement, and other community partners.
  • Participates in interagency meetings, case consultations, and community coalitions to strengthen coordinated systems of care.
  • Facilitates regular team meetings and interdisciplinary case conferences to promote collaborative problem-solving.
  • Promotes continuity of care across service systems, including housing navigation, benefits coordination, and connection to community-based supports when appropriate.
Operations, Budget, and Grant Oversight
  • Assists in developing and managing the CCS program budget in collaboration with Behavioral Health Division leadership.
  • Monitors expenditures to ensure alignment with state funding, grants, and contractual requirements.
  • Oversees program-related purchasing and monitors spending against projections.
  • Identifies funding gaps and assists with grant development, reporting, and performance monitoring activities.
  • Ensures compliance with funder requirements and reporting standards.
Quality Assurance, Data, and Compliance
  • Implements a comprehensive quality assurance framework to ensure consistent service quality across providers and staff.
  • Monitors service delivery metrics and program outcomes to support continuous improvement.
  • Ensures adherence to documentation standards, data reporting requirements, and confidentiality regulations including HIPAA.
  • Oversees accurate and timely documentation in required data systems and maintains client records in accordance with regulatory standards.
  • Prepares reports and program summaries as required by DHS and County leadership.
Recovery-Oriented Service Delivery
  • Leads implementation of the recovery model in service design and delivery.
  • Promotes trauma-informed, culturally responsive, and person-centered approaches across the CCS team.
  • Ensures services reflect a holistic understanding of medical, psychological, vocational, social, familial, daily living, housing stability, and substance use needs.
  • Encourages consumer empowerment and meaningful participant involvement in recovery planning.
Other Duties
Performs other related duties as required or assigned.
  • Master’s degree in social work, Clinical Psychology, or a related human services field, or a Bachelor’s degree with minimum of 5 years of CCS experience.
  • Minimum of two (2) years of professional supervisory or lead experience preferred.
  • Experience working with individuals experiencing mental health challenges, substance use disorders, housing instability, trauma, or justice system involvement is highly desirable.
  • Appointment will be conditional upon successful completion of criminal and caregiver background checks.
  • Possess or be working toward licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Marriage and Family Therapist (MFT), or ability to obtain licensure within two (2) months of hire and maintain thereafter.
  • This position may work both remotely and in-office.
  • An ergonomically correct and confidential workspace, at own expense, is required when working remotely.
  • Employees must provide high-speed internet access at own expense when working remotely.
  • Must have valid driver’s license, an acceptable driving record, and access to private transportation for work-related duties.
  • Must possess and maintain personal automobile insurance in the amount of $100,000/$300,000.
  • Paid experience within a Comprehensive Community Services Program.
  • Demonstrated knowledge of recovery-oriented systems of care and evidence-based practices.
  • Training or experience in:

  • Experience with budget oversight, grant writing, and program performance monitoring.
  • Strong knowledge of local community resources and social service systems.
  • Demonstrated community involvement (e.g., board service, coalitions, professional organizations).
  • Experience conducting workshops, research, professional presentations, or program development initiatives.
  • Fluency in a second language is a plus.
