Job Title: RN Case Manager – Home Health (Field-Based)

Shift: Day Shift | Monday–Friday | 8:00 AM – 5:00 PM (Flexible Schedule Options Available)
Position Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to provide high-quality home health care services in a field-based setting. This role involves managing a caseload of patients in various community environments, delivering skilled nursing care, and ensuring continuity of care through effective coordination and communication.

Key Responsibilities
- Manage and coordinate care for a caseload of approximately 22–25 patients per week
- Conduct 5–6 patient visits daily
- Provide patient and family education on disease management and care plans
- Collaborate with physicians, case managers, and interdisciplinary teams
- Monitor patient progress and adjust care plans as needed
- Ensure timely and accurate documentation in EMR
- Participate in daily team huddles
Patient Population & Clinical Exposure
- High-acuity patients requiring advanced care, including:
- PleurX drains
- PCA pumps
- IV antibiotics (IV ABX)
- Total Parenteral Nutrition (TPN)
- Foley catheters
- Wound care and wound VACs
- Chest tubes and other drainage systems
Required Qualifications
- Minimum 1 year of Home Health RN experience
- First-time travelers accepted
- Active RN License (Wisconsin)
- BLS Certification
- N95 fit test required prior to start
Required Skills
- Strong patient and family education skills
- Effective communication with providers and care teams
- Knowledge of disease management
- Familiarity with troubleshooting:
- PleurX drains
- Nephrostomy tubes
- Tracheostomies
- PCA pumps
- Gastric suctioning
- (Hands-on training will be provided if needed)

- EMR: Epic (experience preferred)
- Equipment provided: Laptop, cell phone, PPE

Schedule & Work Details
- Shift: 8:00 AM – 5:00 PM
- Schedule Options: Monday–Friday or Tuesday–Saturday (flexible)
- Daily Huddle: 7:50 AM (via phone or Teams if traveling)
- Weekend Requirement: None (Saturday availability preferred if needed)
- On-Call: None
- Holidays: None
- Floating: May vary based on patient needs

- Up to 2 weeks, based on experience and comfort level
