Job Title: Travel RN - Home Health

Duration: 03 Months (with possible contract extension)
Work Schedule: Monday – Friday (08:00 AM – 4:30 PM)
Weekly Hour Guarantee: 40 hours
Overview
We are hiring a Registered Nurse (RN) for a Home Health assignment based in Portland, Oregon. This is an onsite, contract role ideal for experienced nurses who enjoy independent case management and traveling within a community setting.
Key Responsibilities
- Case Management: Function as a Skilled Nurse Case Manager, overseeing patient care plans and outcomes.
- Clinical Visits: Perform Start of Care (SOC) visits, Recertifications, Evaluations, Resumptions of Care (ROC), and subsequent skilled nursing visits.
- Documentation: Coordinate visits and ensure all clinical documentation is accurate and submitted timely.
- On-Call: Participate in an on-call and weekend rotation as required by the branch.
Coverage Area
- Location: Primary coverage is within a 60-mile radius of the Salem/Portland area.
- Reimbursement: Mileage is reimbursed for travel between patient visits (from the first patient visit to the last patient visit).
Requirements & Certifications
- Education: Associate’s degree in nursing (minimum).
- Licensure: Active Registered Nurse (RN) License and a valid Driver’s License.
- Certification: Current CPR certification.
- Screening: Must be willing to undergo required occupational health screenings.
Preferred Skills (Nice to Have)
- EMR Experience: History using HCHB (Homecare Homebase).
- Specialized Care: Proficiency in Wound Care.
- Experience: Prior Home Health experience is highly preferred
