Home Health Case Manager Nurse

RN – Home Health Case Manager in Lombard, IL

Shift:

• Monday–Friday, 8:00 AM – 5:00 PM
• 40 hours/week
• Weekend rotation: Every 3rd weekend
• No call, no holidays

Location:

• Coverage: Westchester, Cicero, Berwyn, Oak Brook area (IL)

Weekly Gross: $3165

Breakdown:

W2: $45/hr

Stipend: $1365/Wk

Requirements:

• Minimum 1 year Home Health experience (MANDATORY)
• First-time travelers not accepted
• Active IL or Compact RN License
• Valid Driver’s License & Auto Insurance

Certifications:

• BLS

Patient Details:

• Adults (occasional adolescents)
• 5–6 patients per day

Key Skills:

• Patient assessment & care planning
• Wound care, IVs, venipuncture
• Central line & pleural drain care
• Patient/family education
• Time management & communication

Additional Info:

• EMR: Epic (preferred)
• Equipment: IV pumps, Kangaroo pumps
• Orientation: 1–2 weeks
• Dress code: Professional attire
• Floating: Patient homes within coverage area
