🚨 Travel Opportunity | RN Case Manager or Licensed Social Worker (LCSW/LMSW) – Washington, DC
Looking for your next impactful assignment in care coordination and discharge planning? We’re hiring experienced RN Case Managers and Licensed Social Workers for a high-acuity hospital setting in Washington, DC.
📍 Location: Washington, DC
📅 Duration: 13 Weeks
🕒 Schedule: 4x10 Day Shifts (8:00 AM – 6:30 PM)
💰 Weekly Pay: $2,400 weekly gross ($18/hr on W2 + $1680 weekly per diem)
🏥 What You’ll Do:
You’ll play a key role in ensuring smooth patient transitions and high-quality care coordination across multiple units including ICU, general medicine, surgery, and geriatrics.
Your responsibilities will include:
- Discharge planning and care coordination
- Reviewing admission criteria and insurance benefits
- Conducting patient assessments and coordinating post-acute care
- Arranging placements (home health, hospice, rehab, LTAC, nursing homes)
- Issuing Medicare notices and facilitating compliance processes
✅ What We’re Looking For:
- RN Case Manager OR Licensed Social Worker (LMSW/LCSW/LGSW)
- Active or pending Washington, DC RN license (proof of application accepted) or can hold LMSW/LGSW/LCSW
- BLS Certification
- Minimum 2 years of recent discharge planning experience in acute care
- Prior travel assignment required
- Strong knowledge of CMS, HIPAA, and care coordination workflows
