We are seeking a Project Consultant to lead high-impact initiatives, specifically our Home Modification Program. In this role, you will bridge the gap between clinical recommendations and home-based care, ensuring patients have the physical environment necessary to recover safely and avoid hospital readmissions. You will drive process improvements, manage complex budgets, and act as a key liaison between medical leadership and external vendors.
Roles and Responsibilities
Strategic Program Leadership: Oversee the lifecycle of Home Modification programs, transforming clinical safety needs (e.g., aging-in-place, mobility adaptations) into scalable operational workflows.
Process Engineering & SOPs: Develop and maintain comprehensive Standard Operating Procedures (SOPs) and dashboards to track program health, vendor performance, and patient impact.
Operational Excellence: Facilitate and implement Lean or similar Process Improvement (PI) projects to remove bottlenecks in service delivery and improve business outcomes.
Financial Stewardship: Partner with the Medical Office Controller to monitor program spend, identifying trends and designing remediation plans for budget variances.
Stakeholder Influence: Create and execute tailored communication strategies for a diverse audience, including physicians, health plan representatives, and medical center leadership.
Organizational Redesign: Consult on leadership structure and physician development programs to ensure the organization is staffed to meet evolving patient needs.
Qualifications & Skills
Experience: 5+ years of experience managing medium-to-large scale projects, preferably within a hospital, health plan, or managed care environment.
Healthcare Acumen: Deep understanding of care transitions, Social Determinants of Health (SDOH), or patient discharge planning.
Technical Proficiency: Advanced Excel skills and hands-on experience with Epic Systems (or similar EHR), Tableau, and Smartsheet.
Data-Driven Mindset: Proven ability to translate complex data sets into actionable insights and executive-level reporting.
Collaborative Influence: A track record of successfully influencing senior stakeholders and cross-functional teams to drive large-scale organizational change.
Education: Bachelor’s degree required; PMP, Lean Six Sigma certification, or Master’s in Healthcare Administration (MHA) is a plus.
