JOB SUMMARY:

To oversee and supervise Hospice care provided to the terminally ill patient and their families in all settings, twenty-four hours/day, seven days/week. This role provides clinical leadership, ensures compliance with regulatory standards, and coordinates interdisciplinary care to support physical, emotional, and spiritual needs.

QUALIFICATIONS:

  • Educational/Degree: Graduate from an accredited school of nursing.
  • Training/Licensure: Currently licensed as a registered nurse to practice in Ohio.
  • Knowledge/Skills/Ability: Ability to work independently and make accurate, quick judgments. Ability to supervise staff appropriately and respond to crises outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes.
  • Experience: Minimum 2 years of experience as a registered nurse, preferred. Active patient contact within the past three years, preferred.

CORE RESPONSIBILITIES:

  • Clinical Leadership & Oversight: Develops and maintains clinical standards, policies, and procedures to ensure safe, therapeutically effective service in compliance with Medicare, Medicaid, and Joint Commission regulations.
  • Quality & Compliance (QAPI): Leads Quality Assurance and Performance Improvement projects, chairs Infection Control committees, and monitors publicly reported quality measures to ensure regulatory excellence.
  • Regulatory Documentation: Oversees the accuracy of terminal illness certifications, diagnosis coding, and clinical chart reviews (admissions, recertifications, and discharges) to ensure billing eligibility and audit readiness.
  • Patient & Family Advocacy: Assures the implementation of the Interdisciplinary Group (IDT) plan of care and serves as the primary investigator for patient care concerns or complaints.
  • Operational Management: Manages staffing patterns, 24/7 on-call scheduling, and personnel utilization in coordination with the Administrator.
  • Team Development: Acts as a clinical resource and mentor, overseeing staff performance and professional conduct
