Job Summary and Responsibilities

As our Director of Care Coordination, you will be responsible for leading, facilitating, and implementing strategies to optimize patient care coordination and outcomes across our facilities.

Every day you will oversee the comprehensive care coordination functions, ensuring seamless transitions, efficient resource utilization, and patient-centered approaches. You will work closely with interdisciplinary teams, physicians, and leadership to enhance clinical effectiveness, patient satisfaction, and alignment with system goals.

To be successful in this role, you will demonstrate exceptional leadership, strategic planning, and operational management skills. You will drive continuous improvement initiatives, analyze performance metrics, and ensure adherence to best practices and regulatory standards, ultimately achieving superior clinical and financial outcomes for the organization.

  • Oversees the daily operations of the Care Coordination program, ensuring accountability, effectiveness, efficiency, and compliance with regulatory and accreditation agencies, including direct planning and implementation across the continuum of care.
  • Accepts multiple management tasks to handle financial, operations, managed care & staffing issues within the Care Management departments.
  • Serves as clinical advisor to care management across the continuum of care for all programs, including but not limited to, participation in program development, payer relations, facility relations; patient tracking across the continuum of care; decisions regarding alternatives to acute care which consider quality and financial factors.
  • Analyzes data and develops strategies and action plans to address opportunities for improvement.
  • Develops department and division strategies to ensure efficient operations and goal achievement.
  • Maintains relationships with medical staff members, CNO, nurse VPs, Directors/managers, department managers and technical Directors to support intradepartmental goals and advice on activities pertaining to outcomes.

Job Requirements

  • Bachelors Of Nursing
  • Masters Other in nursing, allied health or business administration
  • Seven (7) years administrative and health care system experience
  • Seven (7) years of clinical and care management experience
  • Five (5) years of leadership experience.
  • Registered Nurse: CA, upon hire
  • Certified Case Manager or Accredited Case Manager, upon hire
