Salary Range: $29.00 To $34.00 (USD) Hourly

Position Summary:

The Clinical Coordinator will join the Dream Youth Clinic multidisciplinary team which

provides holistic wrap-around services for youth ages 13-25 years old. This role requires the

ability to take initiative, multi-task, communicate effectively, and support staff in providing

youth-led programming for our clients. At Dream we believe in youth-led, adult guided

collaboration and are committed to "meeting youth where they are" and assisting them along

their journey to wellness in an open and non-judgmental way.

We are seeking a dedicated and detail-oriented Clinical Coordinator to support the efficient

clinical operations of our Dream’s clinical sites and clinical programming. This role is ideal for a

proactive professional passionate about health equity, patient advocacy, and clinical data systems,

community-based care. The Clinical Coordinator will play a key role in creating and managing

clinic workflows, creating and organizing clinical policies, supporting providers, coordinating

patient care and clinical initiatives, and ensuring compliance with healthcare insurance systems

and regulations.

Key Responsibilities:

Clinic Operations & Patient Care Coordination

● Oversee daily clinical operations to ensure smooth and efficient patient flow.

● Create and coordinate patient scheduling, referrals, and follow-ups to enhance care

continuity.

● Coordinate clinical work-flows, standardization of electronic health records (EHR)

documentation, and care coordination.

● Ensure all clinic protocols align with best practices in youth-centered care.

● Monitor and maintain clinic inventory, supplies, and equipment.

Clinical Programs

● Oversee clinical program delivery and adherence to program deliverables

● Create and coordinate clinical program work-flows, standardization, and integration into


the EHR

● Collaborate with external clinical program partners and represent Dream in external

meetings / partnerships

● Manage and keep track of clinical program benchmarks and deliverables.

Administrative & Quality Improvement

● Assist Dream Data Coordinator with data collection, reporting, and quality improvement

initiatives.

● Ensure compliance with health and safety regulations, including public health reporting.

● Train and support clinic staff on workflow efficiency and EHR best practices.

● Serve as a liaison between clinic staff, community partners, and healthcare organizations.

Patient Advocacy & Community Engagement

● Support uninsured and underinsured youth in accessing healthcare services.

● Collaborate with health advocates and program coordinators to support integration of

clinical programs where applicable.

● Participate in outreach efforts to promote clinic services and youth health education.

● Assist in implementing Dream’s clinical programs related to reproductive health, mental

health, HIV prevention, substance use programming and preventive care, etc.

Direct Clinical Service

● Provide Medical Assistant support on-site in the clinic at least 8 hours / week.

Qualifications & Skills:

● Education: Bachelor’s degree in Public Health, Healthcare Administration, Nursing, or a

related field (or equivalent experience).

● Experience: At least 2 years in a healthcare or community health setting; experience

working with youth populations preferred.

● Strong understanding of healthcare workflows, EHR systems (AthenaHealth preferred),

MediCal and other insurance systems, and clinical programming.

● Strong external liaison skills including collaborating with external partners, building

relationships, and troubleshooting collaborations challenges

● Excellent organizational, written and oral communication, report writing, and

problem-solving skills.

● Growth-mindset, positive outlook focused on creating systems and processes to support

success.

● Ability to work in a fast-paced, collaborative, and trauma-informed care environment.

● Passion for youth health equity and community-based medicine.

Preferred Qualifications:

● Certified Medical Assistant (CMA) with 3 years of experience or Registered Nurse with

1 year of experience

● Knowledge of insurance navigation and social services for youth populations.

● Bilingual in Spanish is a plus


Physical Conditions and other Requirements:

● Frequent standing, stair climbing, bending, stooping, lifting (20 lbs. max).

● Gross and fine manual dexterity.

● Ability to visually observe staff and youth to understand and interpret non-verbal

behavior.

● Personal transportation required.

● Driving to various work locations.

● Ability to operate standard office machines and equipment including computers, computer

software, copy machine, scanner, and keyboards.
