Community Health Care Louisville is looking for a full-time Medical Assistant (MA, CMA) or Licensed Practical Nurse (LPN) with clinical experience to add to our Family Practice team in providing telephone support and triage. Looking for an experienced, energetic, team player with proficient computer skills to build relationships with patients, manage and assist with patient care needs, support decision-making, schedule appointments, create and facilitate telephone encounters, and work collaboratively with the providers and care team to meet the patient’s needs. The telephone intake position consists of 40 hours, 5 days a week.”
Responsibilities:
- Performs phone triage and has direct patient contact to ensure timeliness of return calls to patients and/or family.
- Proficient in managing multiple phone lines and a steady influx of calls.
- Collecting required information from patients to efficiently triage patients based on illness acuity and office processes.
- Providing medical advice to patients who have acute/chronic health conditions as directed by the provider.
- Provide patient education regarding illnesses/diagnoses, symptom management, medications, and treatment plans.
- Schedule appointments.
- Access electronic medical records for patients to review results, medical history, and orders, and document in the patient record.
- Assist care teams with telephone encounters and return calls to patients.
- Assist with medication refills.
- Transfer calls to appropriate staff/care teams.
- Assist medical emergency teams in coordinating and transporting patients to the hospital.
- Assist with referrals to home health care/skilled nursing, transfer of records and orders.
Requirements:

- Analytical skills to assess patient condition.
- Effective oral communication skills to interview patients and effectively communicate with Providers and care team members.
- Strong written communication skills to record patient information clearly and concisely.

- Willingness to adapt and be flexible in stressful situations.
- Ability to collaborate and communicate as a team.
- Familiarity with electronic medical records, word processing, and email software programs.
