The PRN Clinical Pharmacist is responsible for working with physicians, care managers, and other clinical staff, as part of a medical care team focusing on patient outcomes, while maintaining cost.
The Clinical pharmacist provides patients and providers with therapeutic assessments and selected disease management, for assigned practices, working towards improving patient and practice outcomes. The clinical pharmacist also performs other duties consistent with the job classification as required. ADDENDUM to the Job Duties and Qualifications:
Oncology Specific Duties and Credentials
Chemotherapy Drug Review:
Reviews chemotherapy treatment protocol selection for appropriateness based on patient's diagnosis. Calculates and verifies computer generated chemotherapy dosages based on patient specific data. Communicates inconsistencies in dose calculations/computer generated doses with physician. Accurately prepares and dispenses sterile chemotherapy and IV infusions in accordance with USP 797 guidelines. Maintains appropriate patient chemotherapy profiles for duration of active therapy cycles.
EDUCATION QUALIFICATIONS:
Preferred: 2 years of experience of Chemotherapy Dose Calculations, Chemotherapy Patient Assessment, Adult Neonatal TPN preparations, and Neonatal Dose

GENERAL DUTIES
Assists in the development and monitoring of treatment protocols and procedures:
Assists the Clinical Pharmacy Manger in developing and monitoring clinical pharmacy programs. Assists in implementing and revising programs to assure continued success, consistent operation of clinical services, and improved patient care. Continually problem solving. Attends Core Measures planning groups, Pharmacy & Therapeutics Committee, and related Process Improvement groups within the institution. Regular attends and plans for various groups assigned. Assists in the development and implementation of treatment protocols and procedures, clinical initiatives, and guidelines. Assists in updating departmental policies and procedures as dictated by the clinical practice. Participates in MUE and QA programs.
Drug Knowledge Utilization:
Provides clinical consultation, drug information and clarification to practitioners, nursing staff, patients, and family. Sustains hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchange protocols and suggesting appropriate cost effective and therapeutic equivalent alternatives. Is knowledgeable of Louisiana and Federal pharmacy laws and DEA regulations.
Participate in provision of patient-focused services:
Actively participates in the provision of patient-focused clinical therapeutic services. Participates and provides in-service education for medical, hospital, and pharmacy staff. Suggests and implements new clinical practices that would aid in patient care and reduce financial burden. Adheres to current formulary.
Acts as a liaison to other healthcare practitioners:
Serves as a liaison to other healthcare practitioners through participation in interdisciplinary clinical rounds, provision of drug information, and patient family education. Performs and documents timely clinical interventions.
Collaborates with pharmacy staff:
Collaborates with central pharmacy staff to insure timely resolution of problems and questions. Fosters teamwork and takes responsibility to ensure delivery of high-quality care. Provides timely drug information.
Awareness of regulatory standards:
Demonstrates awareness and understanding of record keeping, JCAHO standards, and State Board of Pharmacy requirements. Demonstrates awareness and understanding of departmental policies and health-system initiatives record keeping.
Supervises students and academic associates:
Participates and oversees academic/clinical associate's activities while ensuring compliance with protocols and standards. Effectively develops students and subordinates by recognizing their abilities and weaknesses. Exhibits fairness and impartiality with employees. Provides timely updates of performance standards.
EXPERIENCE QUALIFICATIONS
Required: 1 year residency or 3 years Infectious Disease (UMC)
Preferred: 1 year of experience Hospital pharmacy settings (UMC)
