Clinical Manager

JSSA Hospice has an immediate opening for a Clinical Manager to manage our weekend team. Must be available to work every weekend, from Friday from 10am through Monday 10am. The weekend Clinical Manager is an active mentor and first line resource to the weekend nursing team providing teaching, support, and guidance. This position will work in a hybrid environment, both remote and onsite, and be eligible for full benefits.

Responsibilities:

• Hires, trains and manages weekend LPN and RN staff
• Coordinates weekend visits and admissions coverage
• Performs 90-days, annual competencies and reviews with nursing staff
• Acts as the administrator on-call throughout the weekend
• Serves as a clinical resource for referral sources and patients/family members
• Facilitates end of life conversations and decision-making with the family or caregiver
• Responsible for triaging incoming calls, identifying and coordinating patent/family needs and for guiding staff to provide preventive and supportive care to terminally ill and patient/family units
• Represents our organization when communicating with patients, families, physicians and the community.

Qualifications:

• Active RN License to practice in the State of Maryland
• CHPN certification preferred
• Minimum of 5 years of acute care, Hospice and/or home health required experience
• Supervisory experience required
• Experienced with Electronic Health/Medical Records systems. HCHB experience preferred
• Highly proficient with Microsoft 365 applications
