Trinity Health PACE Alexandria is seeking a compassionate and skilled Certified Occupational Therapy Assistant (COTA) to support our mission of keeping older adults living safely and independently in their homes and community.

The COTA provides participant‑centered occupational therapy services under the supervision of a licensed Occupational Therapist, working within the PACE interdisciplinary team to address functional needs of frail elders through clinic‑based, home‑based, and community services.

Position Purpose

Under the supervision of a licensed Occupational Therapist, the Certified Occupational Therapy Assistant (COTA) provides occupational therapy services to PACE participants to support independence, safety, and quality of life. Services are provided in the PACE center, clinic, participant homes, and community settings in coordination with the Interdisciplinary Team (IDT).

Essential Functions
  • Implements occupational therapy treatment plans as established by the supervising Occupational Therapist
  • Provides therapy services to frail elderly participants in clinical, home, and community environments
  • Assists participants with activities of daily living (ADLs), instrumental activities of daily living (IADLs), functional mobility, and transfers
  • Trains participants and caregivers on adaptive equipment, safety strategies, and energy conservation
  • Observes, reports, and documents participant progress and functional changes
  • Participates in interdisciplinary team meetings and care planning
  • Completes documentation in a timely manner in accordance with CMS PACE regulations, Trinity Health policies, and program standards
  • Supports fall prevention, dementia‑informed care, and participant‑centered services
Minimum Qualifications
  • Graduate of an accredited Occupational Therapy Assistant program
  • Current Louisiana OTA/COTA license in good standing (or eligible)
  • NBCOT certification
  • Valid Louisiana driver’s license and reliable transportation
  • Ability to work effectively with frail elderly participants
  • Strong communication, documentation, and teamwork skills
Preferred Qualifications
  • Experience in geriatrics, PACE, home health, or community‑based care
  • Experience working in interdisciplinary or value‑based care models
  • Familiarity with EMR/EHR systems
Physical Requirements
  • Ability to assist with participant transfers and mobility
  • Frequent standing, walking, bending, and lifting
  • Ability to travel locally between PACE sites and participant homes
