We offer comprehensive services for survivors, including service-enriched housing, nationally accredited child and family development programming, and free counseling to anyone who has experienced domestic violence.
Position Summary:
The Case Manageris responsible fordevelopinga therapeutic relationshipwith theprogram participants,assessstrengths and needs,createsgoals,implementsandmonitorsthe case management plan.Maintain collaboration with landlords in the community.Provides supportive counseling andfacilitatesgroups.
Essential Duties and Responsibilities:
- Completes intake referral interviews with the Assistant Program Coordinator.
- Orient newprogramparticipantsand meet regularly with theparticipantsin the office and in the community.
- Develop a therapeutic relationship, assess strengths andneeds,formulatesa case managementplanandimplementsthat plan.
- Completeandmaintainaccurate, thorough documentation reflecting all case management and counseling functions and activities.
- Communicate essential issues tocollateralcontacts. Gather and incorporate information from other sourcesregardingtheparticipant.
- Identifyresources including housing and linkprogram participantsto a comprehensive set of services. Advocate for services toassisttheparticipants.
- Establish andmaintainprofessionalcontactswith House of Ruth staff and staff from other organizations to ensure coordinated services.
- Accompanyprogram participantsto appointmentsasappropriate.
- Develop and conduct a variety ofpsycho-educationand support groups and activities, teach a range of living skills and social skills including parenting.
- Provide positive role modelingto program participants.
- Provide supportive counseling, problemresolutionand crisis intervention sessions. Intervene during and after an incident.
- Ensurethat theparticipantsadhere to the agreed upon program expectationsand address instances of non-adherence to program policies and procedures.
- Create &maintaina safe,secureand non-violent environment for allprogram participants.
- Effectively manage all emergencies by following standard procedures.
- Monitorsmedications,conducturine toxicologyscreensand complete all related documentation.
- Screen telephone calls and walk-ins,providescrisis intervention andmakeappropriate referrals.
- Provide oncall support/program coveragewhen needed.
- Provide on-going briefings to theAssistant Program Coordinatoron all relevant issues and pertinent informationrelating totheprogram participants.
- Attend all training sessions and meetings.
- Develop,maintainand collaborate with landlords in the community.
- Input andmaintaindatabase system applicable to program.
- Case managersassistandsupportgrant reporting by providing client and case management information to the APC, Data specialist, and the grant manager whenrequested.
- Other duties may be assigned.
Education andExperience:
- Bachelor's degree in social workorrelatedfieldrequired.
- 1-2 years’experience in acasemanagerrole.
- Experienceworkingin a residential settingpreferred.
