Case Manager

This Case Manager (RN) role supports hospital care management by coordinating patient care, facilitating safe and timely discharges, and ensuring continuity of services across care settings. You will conduct comprehensive assessments, develop individualized care plans, collaborate with interdisciplinary teams, and guide patients and caregivers through the transition from hospital to home or other care environments. This is a contract position with a strong focus on acute care, utilization review, and discharge planning in a fast-paced hospital setting.

Responsibilities

• Conduct initial patient evaluations and needs assessments to determine clinical, psychosocial, and discharge planning requirements.
• Develop individualized plans of care that support safe discharge, continuity of services, and long-term health goals.
• Coordinate discharge planning, including arranging home care services, durable medical equipment, and follow-up appointments.
• Collaborate closely with internal care teams, including physicians, nursing, therapy, and social work, to ensure seamless transitions across care settings.
• Work with external providers, community agencies, rehabilitation centers, and long-term care facilities to coordinate ongoing services.
• Perform utilization review and utilization management activities to support appropriate level of care and resource use.
• Complete prior authorizations and related insurance coordination as needed to facilitate timely care and discharge.
• Educate patients and caregivers on care plans, medications, follow-up care, and community resources to support discharge readiness and long-term success.
• Document care delivery, patient response, care coordination activities, and teaching in the Epic electronic health record in a timely and accurate manner.
• Refer patients to appropriate community resources, rehabilitation programs, and long-term care facilities based on assessed needs.
• Participate in care conferences and interdisciplinary planning meetings to align goals and plans across the care team.
• Engage in disability case management activities when appropriate to support patient recovery and return to function.
• Ensure compliance with hospital policies, regulatory requirements, and best practices in case management and discharge planning.
• Demonstrate regular and consistent attendance and reliability in scheduled shifts, including weekend and float assignments.
• Adapt to floating between units and facilities as needed, maintaining high standards of patient care and collaboration in each environment.

• Active Oregon Registered Nurse (RN) license.
• BLS certification through the American Heart Association.
• At least 1 year of case management and/or utilization review experience.
• Recent acute care experience in a hospital setting (behavioral health experience alone does not qualify).
• Proficiency with Epic electronic health record for documentation and care coordination.
• Strong knowledge of discharge planning, care coordination, and community-based services.
• Demonstrated skills in utilization management and utilization review.
• Experience with discharge planning, care planning, and prior authorizations.
• Excellent communication and interpersonal skills to work effectively with patients, caregivers, and interdisciplinary teams.
• Ability to perform thorough needs assessments and develop individualized care plans.
• History of regular and consistent attendance as reported by previous employers.

• Case Management Certification preferred but not required.
• Experience as a Case Manager, Care Manager, or Care Coordinator in an acute care environment.
• Background in disability case management is beneficial.
• Strong organizational and time management skills to handle multiple discharges and care coordination tasks.
• Ability to work independently while collaborating effectively within interdisciplinary teams.
• Comfort with navigating community resources, rehabilitation services, and long-term care options.
• Adaptability to float between units and facilities and work with diverse patient populations and clinical teams.

This role is based in a short-term acute care hospital with approximately 418 staffed beds in Portland, Oregon. The schedule is Monday through Friday, 7:00 AM to 3:00 PM (5x8), with an every-other-weekend requirement. You may be required to float to other units and affiliated facilities, including locations such as Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, and St. Vincent, based on unit needs. When floating for consecutive shifts, hotel accommodations may be offered, and floating to the Seaside facility includes an additional hourly incentive and mileage reimbursement. The environment is fast-paced and highly collaborative, with frequent interdisciplinary interaction and a strong focus on safe, efficient patient throughput and discharge planning. Documentation and care coordination occur in the Epic electronic health record system. Dress code allows any color scrubs or business casual attire, and free parking is available. Job Type & Location

This is a Contract position based out of Portland, OR.

The pay range for this position is $40.00 - $60.00/hr.
