How you'll make an impact in this role

  • Design Person-Centered Care:Build comprehensive transition plans that bridge the gap between hospital and home, ensuring every step reflects the patient's unique needs and goals.
  • Champion Patient Advocacy:Proactively identify and dismantle barriers to care, ensuring your patients have the resources and support systems they deserve.
  • Act as a Clinical Subject Matter Expert:Serve as the go-to resource for the care team on complex issues, including crisis intervention, neglect, and adjustment needs.
  • Drive Care Excellence:Maintain a continuous, collaborative pulse with supervisors and transition teams to refine health plans and ensure seamless implementation.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • One or more of the following required:
    • Clinical Counselor obtained prior to hire date or job transfer date.
    • Bachelor Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
  • Case Manager credentialed from the American Case Management Association (ACMA) preferred.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.

  • LCSW
  • Comfort with handling emotionally difficult conversations
  • Well-organized and proactive
