Work Schedule: Full time, Monday through Friday 8 am to 4 pm (40 hours), with flexibility to meet program needs

A Day in the Life of a Care Manager:

As a dedicated full-time Care Manager, you will play a pivotal role in providing exceptional and comprehensive care management services to adults, supporting their journey. You will be responsible for maintaining contact with members throughout Wayne, Monroe, Ontario and Seneca counties.

  • Provides assistance with housing, transportation, food, clothing and other barriers to optimal health. Drafts and communicates suggested plan of care and discusses at Care/Service Team. Reviews and revises a comprehensive plan of care with the client to meet needs in collaboration with the service team/providers.
  • Provides comprehensive care management including self-management support, health promotion, connection/referrals to providers, community based organizations, social supports, transitions of care, and crisis support with the goal of decreasing barriers to attending appointments and following the plan of care.
  • Provides coordination with Primary/Specialty medical care, acute and outpatient medical, mental health, substance abuse services, and other care managers involved in supporting the client.
  • Provides comprehensive transitional care involving coordination of care and services following critical care events such as emergency department use, hospital inpatient admission and discharge.
  • Provides crisis intervention addressing events such as emergency department visits or inpatient or other crisis events to assure interventions are effective and necessary modifications to plan of care are made.
  • Home and field visits to provide counseling, crisis intervention, referral and advocacy services, coordination of services, and assistance with basic needs.
  • Preparation of all charting, assessment forms, accountability forms and direct service requirements of state and city regulations.
  • For HARP services, provides front line response to members and providers requesting information on or access to Behavioral Health services. The position facilitates access for clinical case management staff to non-emergency care and triage requests as well as urgent and emergency care requests.

Who We Are Looking For:

  • High School Diploma (Adult Care Management/non Specialty)
  • Two years' relevant experience providing direct services to person diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse.
  • A combination of education and experience is acceptable.
  • Computer experience with current working knowledge of windows, Microsoft Office, Excel, Outlook, and Electronic record/documentation software.
  • Must possess a current, valid New York State driver's license that meets the Agency driver standards.

Preferred Qualifications, Skills & Competencies:

  • Bachelor's Degree in human services or related field or a NYS licensed Registered Nurse.
  • Knowledge of Finger Lakes Region, NY resources.
