Requirements
A. Essential Functions - Operations and Leadership
1. Completes daily rounds of assigned houses to ascertain resident conditions and reassess residents on an ongoing basis.
2. Reviews the self-created schedules of staff to ensure compliance with staffing requirements, regulations, and the organization’s policies. Works with the staffing coordinator to monitor the ongoing schedule. Provides assistance to other managers/staffing coordinators as needed in organizing/arranging staff based on staff availability and the needs of individual homes.
3. Works closely with the ADON and DON to address observations/concerns about the care of the residents with the Shahbaz Care Coordinator.
4. Works with the Housekeeping/Laundry Manager to address observations/concerns about housekeeping issues with the Shahbaz Housekeeping Coordinator.
5. Works with the Lead Guide to address observations/concerns about dietary issues with the Shahbaz – Food Coordinator.
6. Discuss observations/concerns about scheduling issues with the Shahbaz Scheduling Coordinator.
7. Reviews and coaches the self-managed team’s decision-making process and provides examples of effective approaches to decision-making.
8. Observes and coaches the relationship between CNAs and Nurses.
9. Places issues or items for team discussion on the team meeting agenda, initial the entry, and seeks feedback regarding the issues/items from the Team Coordinator after the meeting.
10. Reviews and monitors the team’s compliance with in-service requirements.
11. Retains all hiring, counseling, and dismissal decisions of staff within the scope of VMRC practices and policies.
12. Ensures that organizational communications reach direct care and staff.
13. Ensures routine and as-needed nurse meetings are facilitated to effectively communicate with assigned staff.
14. Leads staff interviews, orientation, and in-services.
15. Exhibits leadership by providing excellent customer service to residents, families, visitors, physicians, co-workers, and other clients.
16. Manages time and resources effectively.
17. Administers/directs counseling procedures as necessary.
18. Participates in a wide variety of special projects and compiles a variety of special reports.
B. Essential Functions – Quality Assurance
1. Creates a community of staff commitment to the organization's mission, vision, and values, as well as focuses on providing the highest quality standard of resident-centered services.
2. 24-hour operational management of the assigned houses (utilizing assigned staff, evening/night supervisors, nurse admin on call, etc.) with consideration for and/or application of the organization's mission, budgetary/cost boundary parameters, and regulatory requirements.
3. Recommends, initiates, and implements changes that will enhance care and service programming for Woodland Park.
4. Participates in the development, maintenance, and implementation of the nursing department's Quality Assurance program.
5. Provides oversight to ensure the current Resident Census and Conditions report (672) and Resident Roster report (802) are maintained for each home.
6. Attend regularly scheduled nursing “stand up” and service line meetings, in-services, and nurse management meetings.
7. Participates in "stand-up" meetings with admissions, social services, and the Medical Director as needed.
C. Essential Functions - Resident Centered Care
1. Promotes actions by staff that maintain or enhance each resident's dignity and individual rights, creating a homelike environment.
2. Provides oversight to ensure completion of admissions/discharges.
3. Facilitates relationship with residents and their families/significant others; encourages an ongoing communication concerning the residents' needs and changing health status.
4. Active involvement in the interdisciplinary team associated with care planning and care conference process.
5. Supports and provides oversight to investigate and resolve resident and family problems/concerns.
6. Facilitates, monitors, and evaluates resident plan of care.
7. Directs, evaluates, and supervises all resident care and initiates corrective actions.
8. Champion’s resident-centered service philosophy and initiatives.
D. Essential Functions - Nursing
1. Ensures that nursing care is carried out according to the physician's instructions.
2. Provides support to Clinical Support Team nurses in resident emergency and acute illness situations, transfers, and death procedures.
3. Applies knowledge of state and federal regulatory requirements with respect to all aspects of resident care delivery and clinical record management.
4. Monitors/reviews clinical record, care plans, and charting to ensure documentation standards are maintained and conducts audits as requested (or needed).
5. Oversees the process for wounds, labs, risk assessments, etc.
6. Performs nurse duties as needed.
7. Conducts med pass observations on licensed staff annually.
8. Supports and provides oversight of the MDS (Minimum Data Set) and RAI (Resident Assessment Instrument) process to ensure accurate and timely completion of resident assessments.
E. Supervision
1. Distributes service line workload to maximize team member talents.
2. Holds regularly scheduled team meetings.
3. Provides immediate coaching and problem-solving for all direct reports.
4. Provides opportunities for personal and professional development, including new tasks, related meetings/seminars, and financial support for professional development.
5. Provides personal support and encouragement for each direct report.
