Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

Salary: Starts at $180,000/yr


Requirements to apply:

  • Minimum of 2 years of experience as a Nurse Practitioner or Physician Assistant (internal medicine preferred)
  • At least 1 year of experience working with adults experiencing homelessness
  • Experience managing moderate to severe mental health and substance use disorders
  • Knowledge and application of harm reduction practices
  • Strong collaboration, assessment, and care coordination skills

Job Overview

We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide high-quality medical care within a Recuperative Care Center setting. This role focuses on supporting individuals experiencing homelessness who are transitioning from hospital, emergency department, or clinic care and require short-term medical oversight and stabilization.


Responsibilities include conducting intake assessments, managing and continuing treatment plans, monitoring acute and chronic conditions, and addressing new medical concerns as they arise. The provider will collaborate with primary and specialty care teams, participate in care conferences, support discharge planning, and utilize motivational interviewing and de-escalation techniques. This role also involves identifying barriers to housing, supporting patient goal-setting, and providing education to promote long-term stability and health outcomes.
