Working with your team, our peer mentor will help guide you towards greater self-advocacy, empowerment and personal responsibility by assisting with resources such as: •Housing •Transportation •Employment •Post-secondary education •Recreation and social inclusionNursing Services True wellness encompasses both the physical and behavioral components of health. Through our Link to Wellness program, highly skilled nurses ensure that members have seamless access to routine preventative services while simultaneously breaking down barriers to meet their diverse physical health needs. Our Link to Wellness nursing services include: •Health assessments •Laboratory monitoring •Injections •Education on healthy eating, sleep, and exercise •Diabetes monitoring and nutritional support •Treatment planning and goal development 07 08Mental Health Therapy Our therapists possess extensive experience assisting individuals and their families.
